YOU DO NOT NEED TO PRE-REGISTER FOR THIS CLINIC.

The clinic will be held on Monday, November 16, 2009 from 2:30 pm to 8:00 pm in the Middle School Gym.

Letters were sent home with elementary students and mailed home to parents/guardians of middle school and high school students with this information. Also sent home was a sample copy of a consent form for your child(ren)under 18 years of age that you MUST fill out when you come to the clinic.

All health department clinics are held contingent upon sufficient vaccine supply. If the supply is insufficient, the health department will work with the school to reschedule the clinic.
